 A dental exam and consultation are required before any competent recommendation can be made.

We advise you to visit a dentist for a comprehensive examination and consultation if you are in need of dental care.

 All services at Aroma Dental clinic are provided on a private, fee-for-service basis, as the clinic does not have an agreement with a Primary Care Trust of the National Health insurance Service.

Estimates and proposed management plan.

Once you and your dentist have settled on a course of treatment, we’ll give you a printed copy of the plan. We will keep you updated and talk to you if the plan needs to be altered due to new diagnostic imaging or clinical data. After 90 days, While the associated costs will expire after 3 days.

Before scheduling actual treatment appointments, feel free to ask any questions you may have about your treatment plan.

Forms of Assent

A signed consent form must be completed prior to receiving certain types of treatment. Before any procedures are performed, you will receive a thorough explanation of what to expect during and after the procedure, as well as any potential risks. Either the patient or the dentist can revoke consent to further treatment at any time.

Consent to treat

You accept this Agreement by clicking a button online on our website that indicates consent and agreement to the terms and conditions herein, or by using any service made available through online or offline, including that you authorise the medical staff, nursing staff, and other personnel at this Aroma Dental Clinic, Aroma Dental Clinic Partners, and associated physician locations (collectively, “Aroma Dental”) to provide care and to administer such diagnostic, radiological, and/or therapeutic procedures and treatments as the medical staff determines is necessary or advisable in your care. If you are signing this document on behalf of another person, you acknowledge that you are consenting on behalf of the patient, and you will indicate your relationship to the patient where indicated below. Assignment of Benefits

You accept this Agreement by clicking a button online on our website that indicates consent and agreement to the terms and conditions herein, or by using any service made available through online or offline, including that you hereby irrevocably assign and transfer to Aroma Dental any monies or benefits to which You may be entitled, including benefits or monies from governmental payers, your insurance company, or other third parties who are financially responsible for your medical care, for example, syndicates, banks, factories, companies, or bodies of institutions. And that you authorise and direct Aroma Dental and its physicians, having treated you, to release to such payers or other third parties who are financially responsible for your medical care all information needed (including but not limited to medical records, copies of claims, and itemised bills) to substantiate payment for your medical care and to permit representatives thereof to examine and make duplicates of any and all medical files pertaining to that treatment and care. You also appoint Aroma Dental as your authorised representative to pursue all rights of payment, to ascertain the benefits available, to collect benefits directly from your insurance company, and to appeal denials and proceed against your insurance company in any action, including legal suit, on your behalf if for any reason your insurance company refuses to pay your claim. The appointment shall include all rights to recover attorney’s fees and costs for such action brought by Aroma Dental as your assignee. You further agree to provide information as necessary and to cooperate with Aroma Dental to process and obtain payments. Intermediaries or carriers for the purpose of payment You request that payment of authorised benefits related to your care be assigned to Aroma Dental.

Guarantee of Payment

You accept this Agreement by clicking a button online on our website that indicates your consent and agreement to the terms and conditions herein, or by using any service made available through online or offline, including that you understand that you are financially responsible for all charges not paid by your insurance company, or other third parties who are financially responsible for your medical care, for example, syndicates, banks, factories, companies, or bodies of institutions. , regardless of the reason, including but not limited to failure to pay within 45 days of the invoice date. 

And that you agree to pay all amounts for which you are responsible for medical services rendered in accordance with the rates and terms of the Aroma Dental Clinic. Should the account be referred to an attorney for collection, you will pay reasonable attorney’s fees and collection expenses.

Cost Of Cash Treatment

Payment in full is expected on the day of service. Payment is expected at the time service is rendered, as we do not offer an account-in-arrears facility. 

If your treatment requires a lab fee, you’ll need to bring at least half of the total cost to the session where impressions are obtained. 

At each appointment, you’ll pay a portion of the total cost for services like dental implants and Invisalign®. Treatment fees that include intravenous sedation should be paid in full before the scheduled appointment so that no monetary transactions or signatures are required while the patient is still sedated. 

Please be aware that if you choose to finance your treatment through a finance company, the finance agreement must be accepted and signed before any treatment can begin. Please be aware that financing is only available for treatments over a specific value (not including emergency dental treatment) and that a minimum deposit may be required. 

Please note that a payment of up to 15 percent of the total finance amount will be imposed if you decide to cancel your procedure after signing the agreement forms and financing the costs of that treatment through the finance company. 

We retain the right to request pre-payment for services in special cases. Future appointments may require a time-based deposit from Aroma Dental Clinic. Prepayments can be subtracted from the final bill for medical care.

All of the following methods of payment are acceptable:

Payment is accepted in the form of cash, debit or credit cards, or cashier’s checks (American Express is not accepted). No personal checks will be accepted for payment at Aroma Dental clinic (unless agreed in advance with the practice owner).

Please be aware that we reserve the right to seek reimbursement for any and all costs incurred in referring overdue accounts to a debt collection agency or a small claims court. Before starting any treatment plan, we reserve the right to request payment in full. If the scheduled procedure cannot be completed in the allotted time, we reserve the right to make additional charges based on the actual time spent in surgery and by clinicians.

Appointments that are missed or cancelled at the last minute.

When cancelling an appointment, please give us at least 48 hours’ notice (during normal business hours). When a patient is scheduled for an appointment, we reserve a surgical slot specifically for them. In the event that you miss your scheduled appointment without providing the required notice, you will be responsible for paying a “Failed Appointment Fee.” This charge is meant to compensate the doctor for the time he or she would have spent on the appointment that was missed. There is a fee for a missed appointment that must be paid before a new appointment can be scheduled. Due to the fact that we do not routinely check our e-mail, texts, WhatsApp, or phone messages, we are unable to accept cancellation notices sent in any of these formats. In the case of a missed appointment or cancellation with less than 24 hours’ notice, Aroma Dental Clinic retains the right to collect the full cost of the treatment. We request a minimum of 72 hours’ notice for appointments lasting more than 1 hour. We retain the right to require a deposit of at least 50% in order to schedule subsequent appointments in the event of a missed appointment or late cancellation. Also, it is at the discretion of the principals to reschedule missed or late-cancelled appointments.

Patient Appointment Arrival Delays

We recognize that maybe some patients travel significant distances to reach our clinic, and that delays in some cases may be unavoidable. Be aware that you might be required to reschedule if you’re going to be more than 10 minutes late.

The Information we have About You:

It is crucial that you provide detailed information about your health and the medications you take. It is vital that you inform your dentist of any changes to these conditions. If a patient’s contact information or medical history changes, they must let the clinic know.

Aroma Dental Guarantee

We at Aroma Dental clinic are so confident in the quality of our work that we offer a 12-month guarantee on all fillings, crowns, and bridges if the conditions below hold: 

  • The patient has no further financial obligations to the clinic as a result of the provided care. 
  • Neither an accident nor any trauma or excessive grinding has harmed the restorations. 
  • Our dentists’ instructions for the patient’s aftercare have been strictly adhered to. 
  • Every six months, the patient goes in for a checkup.
  • If your dentist determines that the guarantee on a particular treatment is shorter than 1 year, you will be informed either orally or in writing.

Images and x-rays

taken of you during your dental treatment at Aroma Dental clinic may be used for marketing and scientific activities on the Aroma Dental clinic website and in promotional and educational materials. Your name and identity will not be made public in any way. But if you don’t want us to do that, please let us know.

Use of Your Contact Data

At Aroma Dental Clinic , our patients’ health comes first, which is why we make use of their contact information to keep them up-to-date on all the latest happenings at the clinic and on our latest deals and discounts. Whenever possible, we like to send out reminders to our patients about upcoming appointments as well as other pertinent information. The clinic may follow up with you via phone, text message, email, or regular mail. Please let us know if you do not want to receive communications from the clinic via any of the above methods.

The Complaints Procedure:

Complaints are taken seriously at Aroma Dental Clinic, and we strive to make every effort to ensure that every patient has a positive experience. A simple complaint form is available from the receptionist, or complaints can be made via email. All complaints must be submitted in writing to the “Complaints Manager” and must be specific enough to be processed quickly. Complaints will be acknowledged in writing or via email immediately, and the management will work to find a solution to the problem as soon as possible (usually within 2 weeks). Please inquire at the front desk for details on our formal complaint procedure and policy. In most cases, the management team will acknowledge a complaint and refer the patient on to the treating dentist if the issue is related to the quality of care provided or the cost of care. It is important to note that Aroma Dental clinic assumes no responsibility for the actions of the treating dentist and serves only as a liaison between the patient and the dentist in these situations. You are welcome to contact the Consumer Complaints Service if you are unhappy with our internal resolution of your complaint . If you are still unhappy, you can take your issue to the General Dental syndicate.

Policies prohibiting tolerance of or participation in abusive behavior

We have a strict policy of not accepting patients who are abusive to our dentists or staff, who are disruptive or intoxicated, or who repeatedly miss or cancel their visits (after multiple warnings). Aroma Dental clinic maintains the right to deny service and/or admission under these circumstances.

Promotions

From time to time, Aroma Dental clinic will have promotions and discounts on services. Appointment availability and treatment acceptability are also factors.

Confirmation of agreement with the Terms and Conditions.

You accept this Agreement by clicking a button online on our website that indicates your agreement to the terms and conditions herein, or by using any service made available through online or offline. We reserve the right to modify this Agreement at any time, in our sole discretion, and will give you notice of any such modifications only to the extent required by law. If we make any significant changes, we will let you know through an electronic Communication, such as a link to the updated Agreement on our website or a secure message sent to your online or mobile device if available. The next time you log in to a Service after Aroma Dental has made a material change to this Agreement, you will be asked to accept or reject the terms of the new agreement. If you do not accept the modification, we reserve the right to terminate your access to the Service. By continuing to use the Service after the Agreement has been modified, you agree to be bound by the modified Agreement. Furthermore, Aroma Dental retains complete discretion to alter, enhance, or eliminate any aspect of the Service. If you continue to use the Service after any changes have been made, you will be deemed to have accepted those changes. You should know that Aroma Dental’s written consent is required before any changes you request can take effect. You further agree to be bound by, and to comply with, any and all other agreements or terms and conditions of service with us, including but not limited to any account agreements, signature cards, end-user licence agreements, or applicable laws and regulations. Unless otherwise specified in this Agreement, the terms of any account agreement you have with us will control to the extent that they are inconsistent with the terms of this Agreement.
